Social Security Office set to be at Disaster Assistance Center Wednesday

COLUMBUS, Miss. (WCBI)- The Social Security Office in Columbus will be at the Disaster Assistance Center to help replace personal information. Here’s what you need to know:

Social Security Cards:

The Social Security Office in Columbus will support the Disaster Assistance Center on Wednesday, March 6, from 8 a.mm to 6 p.m. Applications will be taken for replacement social security cards. It is my understanding a receipt will be provided which will have the applicant’s name and social security number.

Acceptable Identification for a replacement Social Security Card

We can accept only certain documents as proof of identity. An acceptable document must be current (not expired) and show your name, identifying information (date of birth or age) and preferably a recent photograph. For example, as proof of identity Social Security must see your:

  • U.S. driver’s license;
  • State-issued non-driver identification card; or
  • U.S. passport.

If you do not have one of these specific documents or you cannot get a replacement for one of them within 10 days, we will ask to see other documents. Any documents submitted, including the following, must be current (not expired) and show your name, identifying information (date of birth or age) and preferably a recent photograph:

  • Employee identification card;
  • School identification card;
  • Health insurance card (not a Medicare card); or
  • U.S. military identification card.
  • Certified copy of medical record (clinic, doctor or hospital) or letter providing extract data from the medical record (A receipt or record maintained by the applicant is not acceptable.)

There is no fee for a replacement Social Security Card.

Birth Certificates:

The Office of Vital Records, Mississippi State Department of Health, will support the Disaster Assistance Center on Wednesday March 6, 10 a.m. to 6 p.m. by taking applications for Birth Certificates. The applications will be processed in their Ridgeland, MS office on Thursday, March 7. This was set up at the request of Cindy Lawrence. On Friday, March 8, the birth certificates will be delivered to the Disaster Assistance Center. The fee for a birth certificate is $17.

Photo ID: What is an acceptable form of Identification?

  • Photo Driver’s License
  • Photo State Issued ID
  • Employment ID
  • School, College or University ID
  • US Military ID
  • Tribal ID
  • Alien Registration/Permanent Resident Card
  • Temporary Resident Card
  • US Passport
  • What if I do not have a photo ID?

If you do not have a photo ID, an immediate family member may order your certificate for you using their photo ID as the applicant. Or you may include two forms of identification from the following list with your application.

  • Social Security Card
  • Utility Bill (showing address)
  • Medicaid Card
  • Snap/EBT card (showing address)
  • Work Identification
  • Veteran Universal Access ID Card
